Self-screen, and do not enter the building if you:
Are experiencing symptoms such as cough, sore throat, nasal or chest congestion, shortness of breath, muscle aches, fever, rash, diarrhea.
Have been in close contact with someone who has had the above symptoms in the last 10 days.

Well Woman Appointment
Complete the Well Woman Intake Form below, as well as the forms that were texted/emailed.
*Failure to complete paperwork before your appointment may result in rescheduling or greatly reduce the time you have with your provider.
Arrive 10 minutes before your appointment time for check-in. If you are unable to complete your paperwork, arrive 20 minutes before your appointment time.
To ensure the most accurate lab results, our practitioner advises that nothing should enter the vagina 48 hours before your appointment. This includes any over-the-counter self-treatments.
For the most accurate STI results, last form of sexual contact should be at least 7 days prior to appointment.
Please call if you need to reschedule due to bleeding/menstruation or if you are experiencing moderate to severe pain. (On a scale of 0-10, those rated 6 and above are encouraged to be seen immediately in an emergency room.)
